I declare that I have taken out civil liability insurance covering material damage and/or bodily injury that my child may cause to others during the activities. I authorize those in charge of the activity to take all the measures they deem necessary in the event of my child's illness or accident. I release the organizer from all liability in the event of theft and/or loss of personal items.
